PLAYER WELFARE

INSURANCE COVER
All Avondale Heights Football Club Players who have paid their club membership and are fully financial, are covered under Bronze level membership  with JLT SPORT (Australian Football National Protection Program)
This cover is paid for by the Avondale Heights Football Club under arrangement with the EDFL and in the interests and welfare of the players. The cover provides basic financial assistance for Non-Medicare Medical Benefits.

If a player needs to make a claim, download the claim form from the link above and fill out the application. The club declaration is to be signed by club secretary. Follow directions in forwarding form to JLT.

Paraplegia and Quadriplegia Cover for players
The Avondale Heights Football Club have upgraded the level of insurance cover for Paraplegia and Quadriplegia from the standard EDFL level of $500,000 to the maximum cover of $1,000,000 as a means of providing a higher level of player welfare for our players. The cover is for all of our teams from Seniors to Under 10’s.

Australian Football National Protection Program explained
When it comes to reimbursement of personal injury claims, some people get confused about Medicare, the Medicare Gap and what is covered by the National Protection Scheme (Australian Football National Protection Program). For information click here

AMBULANCE COVER
The Avondale Heights Football Club does not cover any player medical liability, private insurance, dental or ambulance cover and strongly recommends that all players are members of their own health fund, ambulance fund and wage protection insurance. 
Some levels of private health insurance may include ambulance cover. For further information on how to obtain ambulance cover click here 

INJURY MANAGEMENT
For injury management, discuss with club sports trainers, consult your own medical practitioner , or health worker (Physiotherapist, Osteopath, etc) For further information on injury management click here

CLUB RECOMMENDATIONS AND PLAYER OBLIGATIONS
The Avondale Height Football Club understand the importance of risk management and strongly recommend that all players take out “Work Protection Insurance”

The Avondale Heights Football Club is not liable for the cost of ambulance cover or medical expenses or for the loss of wages due to injuries

By being a player with the Avondale Heights Football Club, you give the club permission to attend to any first aid treatment deemed necessary. This may include the contacting of emergency services and /or transport to a hospital. All ambulance cost and medical expenses is to be paid by the player. The player may be able to claim back a portion of ambulance transport costs through a JLT claim
